The marriage abroad of a Filipino couple or of a Filipino citizen with a foreign national should be reported to and registered with the Philippine Statistics Authority (PSA; formerly known as the National Statistics Office or NSO) through this Embassy. The Report should be registered within 1 year from the date of marriage.

Basic Requirements:

Note: Please do not staple your documents when submitting them to the attending officer/assistant at the consular window.

1. Duly accomplished Report of Marriage forms in four (4) originals (Download Form HERE);
    *Entries must be TYPEWRITTEN or PRINTED LEGIBLY
     *Entries must be at the time of marriage

2. Present original and submit four (4) photocopies of Marriage Certificate/Contract
    a. You may submit either one of the following:
         • Extract from Marriage Registration – international version, apostillized by the Ministry of Foreign Affairs of Belgium/Luxembourg
         • Certified True Copy of Marriage Registration in Dutch or French, with sworn translation in English, apostillized by the Ministry of Foreign Affairs of Belgium/Luxembourg;

3. Present original and submit four (4) photocopies of Birth Certificate of both parties
    a. For Filipino citizens: DFA-authenticated Birth Certificate issued by the PSA
    b. For Foreign Nationals: Extract from Birth Registration - international version, from the Commune/Gemeente (City Hall);

4. Four (4) Passport photocopies of both parties used at the time of marriage; and

5. Report of Marriage fee: €22.50 (to be paid in cash).

Additional Requirements

For delayed registration of marriage (beyond 30 days after marriage):
-Joint Affidavit of Delayed Registration signed by the couple (Download template HERE)
-Notarial fee: €22.50 (to be paid in cash)

For Filipino spouse whose previous marriage was annulled:
-Present original and submit four (4) photocopies of DFA-authenticated Marriage Contract issued by the PSA with the annotation of the annulment

For widowed Filipino spouse:
-Present original and submit four (4) photocopies of DFA-authenticated Death Certificate issued by the PSA or foreign death certificate (international version) of previous spouse

For foreign spouse who was previously married:
-Present original and submit four (4) photocopies of foreign decree/decision of divorce with sworn translation in English

For widowed foreign spouse:
-Present original and submit four (4) photocopies Death Certificate (international version) of deceased previous spouse

Important Instructions in Filling Out the Report of Marriage Form

• Please leave the Date of Registration and Registry Number fields blank. The Embassy will fill out this part.
• Please do not forget to affix by hand the signatures in the field provided for Signature and the date and place of signature at the bottom of the form.
• If filling out the form by hand, please write legibly and please use BLOCK CAPITAL letters.

Once the application is processed, the Embassy forwards the Report of Marriage to PSA in the Philippines. Applicants may retrieve the PSA-certified marriage certificate in the Philippines a minimum of three (3) months after reporting the marriage to the Embassy.
