A Filipino national who urgently needs to return to the Philippines and has lost or damaged his or her valid passport may apply for a Travel Document. Appointment is NOT necessary. Processing time is three (3) working days.
The Travel Document is valid for 30 days from the issuance date and CAN ONLY BE USED FOR ONE-WAY TRAVEL TO THE PHILIPPINES.
The following are the minimum requirements for a Travel Document application:
- Personal appearance
- Duly accomplished application form (available at the Embassy);
- Affidavit of Lost/Mutilated/Damaged Passport;

- Notarial Fee: €25.00
- Original copy of police report (for lost passports);
- Photocopy of old Philippine passport or PSA-issued birth certificate;
- Three passport photographs taken within the last six (6) months (with white background); and
- Processing Fee: €30.00 (to be paid in cash).
Note: The Consular Officer reserves the right to require additional documents pursuant to the Philippine Passport Law and the Foreign Service Act.
