(Spouses of Filipino Nationals)
Requirements:
- Three sets of duly filled-out and originally signed application form [Click here for Application Form]
- Original police clearance issued by the police in the commune where the applicant resides, with two photocopies
- Original medical examination form duly filled-out by a physician [Click here for Medical Examination Form], including laboratory results and analysis, with two photocopies
- Proof of financial assets (e.g., certificate of land titles, recent bank statement, certificate of stocks, etc.), with two photocopies
- Filipino spouse’s affidavit, with a photocopy of his/her valid passport, indicating he/she is married to applicant and the reasons for migration, with two photocopies
- Original copy of marriage contract issued by the Philippines Statistics Authority (formerly NSO) and authenticated by the Department of Foreign Affairs, with two photocopies
- Three photocopies of applicant’s identification card
- Other requirements not listed above, if required by the consular office
